Need a new physical? Download and print the TCAPS physical form here, and then please call your family physician to schedule a physical for your child.

Sports physicals must be dated April 15th, 2021 or newer, and can be used for the entire 21-22 school year. Sports Physical: Middle school athletes must have an updated physical on file each school year.Log in and select all of the sports you are interested in joining for next year to ensure you are receiving all of the communications from our staff.
